Referral

If Occupational Therapy is required for you, your family member or your participant please complete the referral form and send it via email. The occupational therapist will be in contact with you upon receive of referral. 
*A Service Agreement will be sent after referral submission.
Contact is made in 3 business days after submission of referral.
